Federation Star Ships are navigated using the degrees of the equator of Earth and the two polarities relative to the center of the Galaxy. Every different government in Star Trek uses it's capital as a reference point, so Romulan navigation and Federation navigation work differently. (See: Geography for stellar cartography).
The angle is input by a Helm officer (TOS) or a Comm (TNG, VGR, DS9).
